Translation
Obv.
1–2 I hit you (the fly?) at the cranium,
3–4 from the cranium to the forehead,
5–6 from the forehead to the ear,
7–8 from the ear to the nostril of the nose.
9 I bind you by the oath of Ninkarrak:
10–12 You must rise a locust's rise in his twinkling.

Commentary
General: The different body parts described in the incantation are, contra van Dijk YOS 11, p. 21, not of the fly, but of the man who is trying to get rid of the disturbing insect.
12: Difficult. Thomsen (2018, 773) reads i-na za-ab-ri-šu and translates "von seiner Seite(?)". We prefer i-na ṣa-ab-ri-šu having in mind ṣabrum ("blinker, winker, on who blinks, winks", CDA 331) which could be a fly swatter.
