Translation
12 ... Lamaštu.
13–15 Its ritual: You strike(?) the top of bite with a leather-…;
16–17 You take <dirt?> from the foundation wall of a tavern;
18–19 You smear it on the bite;
19–21 You draw near, you bandage her(sic) – and he will get better.
22 Incantation (against the bite of a) dog.
Commentary
General: Following A. R. George (private communication, April 2014), the incantation is against a dog (kalbi written UR.IGI for UR.GI), not “bedchamber” as suggested by Hallo. Still, the appearance of Lamaštu in l. 12 (if reading is correct) needs explanation.
15: ta-na-aṭ-ṭù-ma: not from nadû, but naṭû "to beat, whip".
16: The key ingredient appears to have been omitted by mistake: probably dirt, or tin. – One expects asurî bīt aštammi
